Abstract

Congenital heart defects occur with a frequency of 0.8 % in newborns. Correction of these heart and vascular malformations is made possible by the use of a heart-lung machine (HLM), which ensures the maintenance of body perfusion and body oxygenation. However, this extracorporeal circulation can lead to an acute systemic inflammatory reaction and damage to the coagulation system, especially in small newborns. These patients suffer postoperatively from fever and dysfunctions of various organ systems up to multiorgan failure.

Commercially available systems consist of only miniaturized individual components of HLM for adults. However, there are technical and physical limits to this concept of scaling, e.g. by the required gas exchange surface. In order to design an HLM specifically for use in premature and newborn children and to significantly reduce the above-mentioned reactions and complications, completely new design approaches must therefore be pursued.

At the Chair of Applied Medical Technology, Helmholtz Institute, RWTH Aachen University, a novel miniaturized heart-lung machine (MiniHLM) was developed by Dipl.-Ing. Jutta Arens in cooperation with the Department of Pediatric Heart Surgery at the University Hospital Aachen as part of her doctorate.
